PAYMENT / FEES
My practice operates on a self-pay, fee-for service basis. This means that your fee for each appointment will be due at the service. Office fees for buprenorphine treatment may be paid by Credit/Debit Card, Money Order or Cash. Fees are as follows:
Initial Appointment: (Physical Exam, Screening, Assessment, and Instructions) approximately sixty minutes. $350
Follow Up Appointments: (Compliance, Assessment) approximately 15 minutes. $225
Urine Drug Screens: (Required each visit) Billed separately. Most insurance is accepted .
INSURANCE
In order to protect patient privacy we do not accept insurance. We will be happy to give you a super bill so that you may file for reimbursement with your insurance company.
CANCELLATIONS
When you make an appointment with me, it is time that I reserve exclusively for you. Barring emergencies, I will be ready to see you at our scheduled time. I do not double-book or over-book my schedule.
Because of this, I ask that you provide me with as much notice as possible should you need to cancel or change an appointment, by calling my office.
My cancellation policy is as follows:
Cancellation 24 hours or more before appointment: No charge
Cancellation less than 24 hours before appointment: $75.00
Cancellation without notice (no-show): $100.00
The associated fee must be paid prior to rescheduling an appointment.
